HLGP-A and RHLGP-A or HLGP-D and RHLGP-D gas pressure switches manufactured between the dates of September – October, 2000 (date code 3800, 3900, 4000, 4100, 4200, 4300, 4400, or 4500).
On 10/12/00 you purchased five Antunes Controls gas pressure switch, 804114202 HLGP-D from Cocrane Supply on your PO# 0039003. We, at Antunes Controls, would like to bring to your attention to a possible problem with our HLGP-A and RHLGP-A and the HLGP-D and RHLGP-D gas pressure switches manufactured between the dates of mid – September – October, 2000 (date code 3800 3900, 4000, 4100, 4200, 4300, 4400 or 4500). Some units manufactured during that period have experienced an internal leak in the castings from the Low gas pressure chamber to the High gas pressure chamber. Depending on your installation, the Low gas pressure chamber could leak into the High gas pressure chamber and by-pass the safety shut off valves. This condition can cause a lock-out by the high pressure switch and/or allow some gas to be introduced into the gas piping system downstream of the switch through the .020” orifice in the high pressure inlet connection. While we are unaware of any incidents of external gas leaks caused by this condition, all potentially affected units must be replaced immediately to prevent this possibility.
The date code is located on the rear of the electrical housing printed in red. The first two digits indicate the week and the second two digits indicate the year. (See enclosed information.)
Immediately inspect all of your installations using our HLGP-A and RHLGP-A or HLGP-D and RHLGP-D gas pressure switches. If the date code on the back of the switch is 3800, 3900, 4000, 4100, 4200, 4300, 4400, or 4500, replace the switch with a new switch.
Return the replaced switch to the following address:
Attn: Double Gas Castings Returns
180 Kehoe Blvd.

We will replace all returned switches free of charge. We will pay up to $50.00 for replacement cost. Antunes Controls will not pay for inspection costs.
