FLUID POWER ACTUATORS RECALL REMINDER
In November 1998, Honeywell announced the recall of V4055, V4062 and V9055 Fluid Power Actuators with manufacturing date codes of 9744 through 9841. We determined that these actuators may have a manufacturing defect which could result in delayed closure of gas valves, possibly creating the risk of a combustion side explosion.
Thankfully, by proactive communication of the potential danger and the publishing and implementation of a safety check-out procedure for start-up, serious damage and injury have been avoided. The check-out procedure allowed many users to continue to operate safely until replacement units were available. However, all users must be aware that it is still necessary to change-out the affected recalled actuators to avoid potential unsafe conditions in the future.
We would like to reiterate that it is still a priority to identify the recalled actuators and have them changed out as soon as possible. Do not continue to operate any longer than necessary, or re-start equipment that has been shutdown without following our attached check-out addendum. Replacement units are available and change-out can be accomplished quickly. Always use the installation and instruction start-up procedure when restarting a unit
